Resident Agreement
- Admission and Enrollment: Residents agree to comply with all admission requirements and enrollment procedures of Gracefields Elderly & Nursing Care Home Service.
- Payment and Fees: Residents agree to pay all fees and charges associated with their stay at Gracefields in accordance with the agreed-upon terms and payment schedule.
- Rights and Responsibilities: Residents have the right to receive respectful, compassionate care and to participate in decisions regarding their care and services. Residents also have the responsibility to abide by the rules and regulations of Gracefields and to treat staff and fellow residents with respect and courtesy.
- Privacy and Confidentiality: Residents' personal information and medical records will be treated with the utmost confidentiality and will only be disclosed as necessary for the provision of care or as required by law.
Visitor Agreement
- Visitation Policies: Visitors agree to abide by all visitation policies and procedures of Gracefields Elderly & Nursing Care Home Service, including visiting hours and restrictions on the number of visitors per resident.
- Respect for Residents: Visitors agree to respect the privacy, rights, and preferences of residents and to conduct themselves in a manner that does not disrupt the peace or well-being of others.
- Compliance with Policies: Visitors agree to comply with all policies and regulations of Gracefields, including those related to safety, security, and infection control.
General Terms
- Use of Facilities: All users of Gracefields facilities and resources agree to use them responsibly and in accordance with their intended purpose. Users are responsible for any damage caused by misuse or negligence.
- Safety and Security: Users agree to comply with all safety and security protocols of Gracefields, including emergency procedures and evacuation plans.
- Liability: Gracefields Elderly & Nursing Care Home Service is not liable for any loss, damage, injury, or harm incurred by residents, visitors, or staff unless caused by gross negligence or willful misconduct.
- Changes to Terms: Gracefields reserves the right to amend or update these Terms and Conditions at any time. Any changes will be effective immediately upon posting on our website or notification to users.
